RRYHA Submission of Forms and Applications
Forms and applications can be submitted via email to the appropriate Hockey Operations Coordinator (HOC).
By Email:
- Youth HOC: Chummer Denny

All forms must be filled out completely and include any required signatures before being submitted.

Player Move-Up Request Process
If you feel your child should be placed on a team at a higher level than their birth year recommends, please complete one of the applicable Player Move-Up Request Forms. Your request will be reviewed by the appropriate RRYHA Hockey Operations Committee (HOC), and you will receive a response as soon as possible.
- In the meantime, please have your player continue attending practices at the level they are currently registered for until a decision is made.
- If the request is approved, the HOC will contact you, and you will be sent a registration link for the adjusted fee associated with the new level of play.
This form must be submitted to the appropriate HOC office before October 1st.

Player Move-Down Request Process
In accordance with USA Hockey and Minnesota Hockey guidelines
Basic Policy:
Players are expected to participate within their designated age group based on their birth year. However, in exceptional circumstances involving documented physical or mental disabilities, athletes may be considered for placement in a lower age division.
Important:
- Play down requests are based solely on documented medical necessity.
- Requests for reasons such as player size, experience level, convenience, or to gain a competitive advantage will not be approved.
- All approved move-down players must participate at the lowest available competitive level within RRYHA.
Request Procedure:
- Parent/Guardian Initiation:
- The request must be initiated by the player’s parent or legal guardian.
- Medical Documentation:
- Submit a written statement from a licensed physician that:
- Describes the player’s disability.
- Explains the increased risks of participating at their designated age division.
- States that, in the physician’s opinion, the player can safely participate in a contact sport such as hockey within a lower age group.
Player Information:
- Include a document with:
- The player’s birth date, current height, and weight.
RRYHA Approval:
- Obtain a letter of approval from the RRYHA Board on official association letterhead, confirming:
- The association’s review and support of the play-down request.
- The lowest competitive classification available within RRYHA for the player to participate.
- Submission to Minnesota Hockey District 12:
- All documentation must be submitted in original hard copy form with signatures.
- No emailed copies will be accepted.
- The complete request package must be sent to the District 12 Director.
- If approved, the District Director will forward the documentation to the USA Hockey Minnesota District Registrar for final approval.
Final Decision:
- The decision of the Minnesota District Registrar is final. There is no appeal process for play-down requests.

Player Waiver Release Request Process
If a player requires a Minnesota Hockey Waiver Release Form, please note that this form is not available for direct download. It must be obtained through your District 12 Director directly.
To request a waiver:
